powerPOWER MISSION

POWER (Peer Outreach Worker Engagement and Retention) is dedicated to improving the health of HIV+ women in Brooklyn. By pairing Outreach Workers with FACES Network case management services POWER supports patient engagement in HIV medical care and supportive services at The HEAT Program, FACES Program, Interfaith Medical Center, The Brooklyn Hospital Center's PATH Center, and Housing Works.

WHAT WE DO

In 2009 the FACES Network received a Health Resources and Services Administration Special Project of National Significance grant to launch the POWER Program. The Outreach Workers employed by the POWER Program work with the FACES Network case managers to guide newly diagnosed women into care, provide mentorship to women transitioning from adolescent to adult care, facilitate support groups, and seek to re-engage HIV+ women who have fallen out of care.

POWER PROGRAM GOALS

POWER will improve the health outcomes of women in Brooklyn who are HIV+ by:
1. Ensuring timely entry into care for newly diagnosed women;
2. Increasing the retention rates of women already receiving HIV medical care;
3. Re-linking women to care who were previously in care, but have been lost to follow-up.
