PRIMARY CARE

HEAT provides comprehensive medical, case management, HIV/STI counseling and testing, PrEP/PEP, mental health counseling, treatment adherence, supportive services and access to research for high-risk and HIV infected youth. HEAT’s services are provided in a culturally competent, youth-friendly environment. We tailor services to the needs of each individual, with awareness around issues of cultural stigma, gender identity, sexual orientation, parenting, and HIV status. Staff on call 24 hours 7 days a week. Did you know that The HEAT Clinic at SUNY Downstate now has a new program specifically geared to TGNC Youth and young adults ages 13-29? It's called THE BLOSSOM PROJECT! THE BLOSSOM PROJECT provides tailored programing for the transgender / Gender Non-conforming community: Primary Care Chest/Breast Health Hormone Care Pelvic Wellness Exams; PAP Tests Sexually Transmitted Infection (STI) Screenings & Treatment HIV/AIDS Testing, Treatment & Prevention, including PrEP and PEP Harm Reduction Counseling Care Coordination Provided by Transgender staff Mental Health Health and Wellness Workshops Surgery Referrals Assistance with Legal Name Change /Gender Marker and more For Further Information Please Contact our Health Service Coordinator at (718) 613-8554.

CASE MANAGEMENT
& SUPPORTIVE CASES

Case Management & Supportive Services Need help with finding a job, housing, health insurance, going back to school or getting your GED? We’re here to help. Case Management is when you have a one on one relationship with a person who helps get you moving in the right direction. For example, you want to get your GED but don’t know where to start. They will give you the tools in order to take the steps in the right direction. Your case manager may refer you to a program that helps you obtain your GED, or they may make a call to a contact in order to register you to take the test. However it’s your duty to make sure you go to your appointments on time, study for the test and get a good grade.
